Drugs are estimated to cause about 10 - 25% of all cases of gynecomastia. Over the course of several decades, multiple medications have been implicated in the development of gynecomastia mostly in the form of case reports and case series. However, these reports suffer from a multitude of deficiencies, including poor quality of evidence. Areas covered: Studies were selected for this review by performing an extensive electronic and hand-search using BIOSIS, EMBASE and Medline, from 1940 to present, for all reported drug associations of gynecomastia and their possible pathophysiology. Quality of evidence was assessed on a three-point scale: good, fair and poor, and each of the drugs reported to cause gynecomastia was assigned a level of strength.

The pathophysiology of gynecomastia is also discussed in detail for each of the drugs found to have a good or fair evidence of association with gynecomastia. Expert opinion: Most of the reported drug-gynecomastia associations were based on poor quality evidence.

The drugs definitely associated with the onset of gynecomastia are spironolactone, cimetidine, ketoconazole, hGH, estrogens, hCG, anti-androgens, GnRH analogs and 5-alpha reductase inhibitors. Medications probably associated with gynecomastia include risperidone, verapamil, nifedipine, omeprazole, alkylating agents, HIV medications (efavirenz), anabolic steroids, alcohol and opioids.

Higher Cortisol Increased Sexual Arousal in Men

A total of 99 men provided a baseline saliva sample, imagined and wrote about a sexual or control situation, and provided a second saliva sample 15 min later. Results indicated that, for participants in the sexual condition, higher baseline and post-activity cortisol corresponded to larger increases in self- reported sexual and autonomic arousal. Although sexual thoughts increased sexual arousal, they did not change testosterone or cortisol compared to control conditions. Our results suggest that sexual thoughts are not sufficient to change testosterone or cortisol in men, but cortisol may facilitate sexual arousal by directing energy towards a sexual situation.

Note: Very high levels of cortisol can decrease testosterone blood levels and may decrease arousal.  The cortisol levels detected in this study are moderate.

Testosterone Therapy Decreases Liver Fat



Testosterone administration in older men with mobility limitation and low testosterone levels was not associated with a reduction in hepatic fat.

Nonalcoholic fatty liver disease is now the most common cause of chronic liver disease in the U.S. Some people with excess fat in the liver simply have what's called a fatty liver. Although this is not normal, it is not serious if it doesn't lead to inflammation or damage.
Others have what's called nonalcoholic steatohepatisis (NASH). Although it is similar to alcoholic liver disease, people with this type of fatty liver disease drink little or no alcohol. NASH can lead to permanent liver damage. The liver may enlarge and, over time, liver cells may be replaced by scar tissue. This is called cirrhosis. The liver can't work right and you may develop liver failureliver cancer, and liver-related death. NASH is one of the leading causes of cirrhosis.
Both types of NAFLD are becoming more common. Up to 20% of adults may have either fatty liver or NASH. And more than 6 million children have one of these conditions, which are most common in Asian and Hispanic children. Recent evidence indicates that NAFLD increases the risk of heart disease in children who are overweight or obese.
